Outreach Clinic

Doctors, nurse practitioners, health professionals and case coordinators serve patients who have difficulty accessing health services because of homelessness, drug use and/or mental health problems.

The team offers primary health care, as well as information and referrals to other community services.

Services are offered through our outreach clinic, as well as shelters and other community programs.

Services include:

  • vaccinations
  • wound care
  • respiratory care and testing
  • foot care
  • HIV, STD and pregnancy tests
  • service coordination
  • information and referrals

Common issues include:

  • asthma
  • hepatitis C
  • HIV/AIDS
  • birth control and pregnancy
  • nutrition
  • foot care
  • mental health
  • pain management
  • addiction

Related issues may include:

  • housing
  • income security
  • literacy and language
  • family and relationships

Eligibility:

People who have difficulty accessing conventional health and social services because of drug use, homelessness and/or mental health problems.

Service boundaries: Don Valley Parkway to Victoria Park Avenue; Eglinton Avenue to Lake Ontario

Tuesdays and Thursday, 2 p.m. to 4 p.m.
